Job title: Clinical Case Manager (Part time and full-time positions available)

Access Institute for Psychological Services is a community-based mental health care nonprofit based in San Francisco that delivers comprehensive and affordable mental health care to those with the greatest need and the least access. For over 17 years, the organization has made high-quality psychological services available to individuals and families who lack adequate insurance and the financial means to afford care. Every year over 13,000 hours of services are provided to more than 700 people of all ages. Access Institute’s clinical training programs (Doctoral Psychology Internship and Post-graduate Fellowship) train the next generation of mental health professionals in contemporary psychodynamic treatment approaches that integrate socio-cultural perspectives and instill ethics of community service. Access Institute provides mental health care at its Hayes Valley clinic and at community locations, including seven public elementary schools and an adult day health center in Bayview Hunter’s Point. 

Position Overview: Access Institute seeks a dynamic and passionate individual to join its team as Clinical Case Manager (CCM). The CCM provides direct service within Access Institute’s Assertive Social Assistance Program (ASAP). This program works to integrate mental health (assessment, therapy, psychiatry) and case management services to keep individuals out of crisis, avoid hospitalization, maintain their housing, and support their long-term recovery. The program serves low-income, marginalized adult individuals and families who lack sufficient insurance or are not being adequately served by the publicly funded system. The CCM provides intensive, clinical case management services directly to clients, provides integrated psychotherapy and case management care, and provides consultation to the rest of the team of mental health providers at Access Institute (therapy trainees, staff psychologists, and psychiatrists). The CCM develops relationships with the key members of the social service network in San Francisco in order to enhance coordination of care for all patients.

The right candidate is a licensed clinical social worker with proven experience in psychotherapy, case management, and community-focused care. We are looking for a highly organized, strategic thinker, who has solid clinical, leadership, and communication skills and enjoys building relationships with individuals and groups.

Essential job functions:
  • Provide psychotherapy and intensive case management services to adults and families. Services are provided at Access Institute’s Hayes Valley Clinic.
  • Coordinate care, assisting patients in accessing housing, financial, medical, and other needed services.
  • Provide case management consultations to Access Institute therapists, psychiatrist, and supervisors.
  • Provide consultation and support to the entire clinical team with the goal of enhancing coordination of care and improving patient care. 
  • Serve as a liaison between Access Institute and social service providers in the community.

Required qualifications:
  • Commitment to Access Institute’s mission and to the cause of affordable, accessible mental health care
  • Experience providing clinical case management services in community-based settings
  • Ability to integrate thinking regarding clinical case management and mental health treatment
  • Licensed in California
  • Experience collaborating with multi-disciplinary team
  • Excellent administrative and organizational skills
  • Strong communication, relationship-building, and leadership skills
